Why a standard panel often stops at TSH

Reimbursement decides the shape of most lab orders. Coverage rules generally tie each additional marker to a documented indication, and a 6-marker thyroid panel ordered for a general look at how things sit often falls outside them. That is a structural constraint on what insurance will pay for, and it sits between the physician and the lab rather than inside the exam room.
