The amount of insulin circulating in your blood after an overnight fast - how much your pancreas is releasing to hold glucose where it currently sits. Routine panels rarely include it.

Fasting glucose

The concentration of sugar in your blood at that same fasted moment. It is the number most people have seen before, and on its own it describes one variable in a 2-variable system.

HbA1c

The share of your hemoglobin carrying attached glucose, which reflects average blood sugar across roughly the previous three months rather than the morning of your draw.

Uric acid

A waste product left over when your body breaks down purines, cleared through your kidneys. It comes off the same draw and your care team reads it beside the other markers.

HOMA-IR

HOMA-IR isn't a sixth vial. It's arithmetic performed on 2 of the markers above - your fasting insulin and your fasting glucose, multiplied and scaled into a single figure that describes the relationship between them. On ordering the test yourself

You can buy a fasting insulin test online for less than lunch, and you'll receive a number and a reference range. What you won't receive is the person who reads it against your glucose, your A1c, your thyroid panel and your history. That reading is the thing you're paying for here. The vial is logistics.

On your existing doctor

Keep them. This isn't a replacement for primary care, and your care team will send your results wherever you'd like them sent. The reason fasting insulin rarely appears on a standard order has to do with what insurers reimburse and when, not with the judgment of the clinician who wrote it.
