Evidence before enthusiasm

Dr. Jacobs co-authored the American College of Physicians evidence guide to complementary and alternative therapies, graded with the same system the World Health Organization and Cochrane use. He has also published trials showing that some of the best-selling supplements in America performed no better than placebo. That standard shapes how your care team decides what belongs in your plan - and what comes out of it.

Remote care, held to clinical standards

In 2005 Dr. Jacobs led one of the first randomized controlled trials conducted entirely online: 391 participants across 45 states, recruited, consented, randomized and measured at a distance. Rigorous care delivered remotely is not new territory for him. He helped prove it was possible.

A conversation with Dr. Jacobs

In plain terms, what do you do?

Precision integrative medicine. I look at the whole person and the whole dataset, then I go where the evidence leads - whether that points to a prescription, a change in how someone sleeps, or telling them plainly that no supplement is going to fix a life they haven't changed.

What do most people never get tested for?

Fasting insulin. Testosterone trajectory over time, not a single reading. Triglyceride-to-HDL ratio. High-sensitivity CRP. Sleep quality. Heart rate variability. VO2 max. None of these is exotic. Almost none of them is routinely ordered because the reimbursement model is built to detect disease that has already arrived, not to watch the direction someone is heading.

You've published research showing popular supplements didn't outperform placebo. Why does that matter here?

In 2005 my colleagues and I ran a randomized, placebo-controlled trial of kava and valerian - two of the best-selling supplements in the country - with 391 participants across 45 states. Neither performed better than placebo. Published research tells you what tends to be true across a population. It can't tell you what's true for you. That's the whole argument for measuring first and deciding second, with someone qualified reading the results alongside you.

Why did you join EverSpan Life's advisory board?

I run a concierge practice in San Francisco. Most people will never walk through that door - not because the medicine is complicated, but because of geography and cost. The biology doesn't care where you live or what you earn. The opportunity to catch something early is the same. Access never has been.
